Register

New Patient Registration

 

If you wish to register with the Practice you can check to see if your address is within the Practice catchment are here: Practice Area

To register with the Practice you will need to complete the online registration form or alternatively print and complete the GP Registration Form.pdf.

You will also need to complete the New_registration_form_.docx 

Forms are also available from Reception.

Once you have completed the registration form, please provide the Practice with two forms of identification; 1 item must confirm your address, e.g. utility bill, council tax bill, driving licence.

Once you have been accepted as a patient your medical records will be transfer to the Practice.

Temporary Registration

 

You can register as a temporary resident with a GP surgery for up to 3 months. 

To register as a temporary patient simply contact the practice to obtain the form (GMS3) that you can get from reception or download here. Once you have completed the form, please hand it into the receptionist and they will be able to advise you further.

You cannot register as a temporary patient in the town or area where you are already registered.

Named Accountable GP

 

The Government requires GP Practice to allocate a Named Accountable GP, to all their patients.

All our patients have been allocated a Named Accountable GP, but you can continue to see ang GP of choice within the Practice.

If you wish to know the name of your Accountable GP please ask when you are next in contact with the Practice.

 

Homeless Patients

 

Homeless patients are entitled to register with a GP using a temporary address, which may be a friend's address or a day centre. The practice address may also be used to register.

 

The Practice has a non-discrimination policy for accepting new Patients. There are a number of reasons why you may not be able to register with the Practice, for example we do not register patients outside of our catchment area.
